Most accountability problems get blamed on people — someone not stepping up, the owner, the team. They're usually something quieter: no clear line for who owns what, what needs a sign-off, and where a decision actually sits.

When that line is fuzzy, things drift in one of two directions. Sometimes everything routes back to the owner or the managers, because letting go feels like letting it slip. Sometimes things run loose with too little direction, and quietly go sideways. Plenty of leaders do both at once — white-knuckling the things they should hand off, while the things they did hand off run unwatched because they're too busy doing everything else.

Leadership & Accountability is the work of making ownership clear — not just for the owner or a team leader, but across the whole team — so the right things get owned at the right level, and follow-through stops depending on one person holding it all together.
